Healtho5 Solutions Private Limited is a Bangalore and Coimbatore based company established in the year 2015 with an aim to revolutionize healthcare by providing easy to use tools to healthcare providers to empower and engage their patients.
This policy tells you what information we collect from you, what do we do with it, who has the access to the information and what you can do about it.
We only collect the information needed to serve you and your patients better. We receive patients detail from the hospital which includes both personal and medical information of the patients with an aim to serve them better. We also collect information through our website and other social sites visited by you. The information collected is basic ranging how you visited the website, where you navigated within the website and what services/ information you were looking for.
The information about your navigation on our website helps us in providing you the best services suitable for your organization and patients. We never collect any payment information like credit card or bank details on our website or MedEngage® platform.
We use the information provided by the patients to serve them better by maintaining longitudinal health record of the patient. The information collected is both personal identifiable information and sensitive information (as defined under SPI rule). Your personal information is never collected or shared without your permission and consent.
Healtho5 Solutions will use the anonymized patient details (after removal of identifiable patient data) for research purposes and to generate business and clinical intelligence report for our clients.
What information is collected by Healtho5 Solutions:
The information collected through the website and other social sites are restricted to your name and contact information that is required to serve you better. We never collect the payment information like credit card information or bank account details.
We provide patient engagement services both as a platform and service (with trained nurses and doctors) with our MedEngage® CRM platform.
The information collected during the phone call is an essential requirement to evaluate patients health when they are out of the healthcare facility. The information collected and stored is needed to provide the best care to the patients.
When we ask you to provide the details through any mode such as email, telephone calls or other correspondence, we may ask you to voluntarily provide us with certain information that personally identifies you or could be used to personally identify you. All the information is voluntarily provided by you and we won’t force you for any information if you do not want to share.
The information collected by us from you may include but is not limited to the following:
“Personal Information” is defined under the SPI Rules to mean any information that relates to a natural person, which either directly or indirectly, in combination with other information available or likely to be available to a body corporate, is capable of identifying such person.

The information provided by patients including personal information or any sensitive personal data or information is voluntary. Any patient who do not want to share any information at any point of time have an option to discontinue the services provided by Healtho5 Solutions immediately.
We at Healtho5 Solutions are not going to disclose any personal information which identifies any individual. We are serious about data privacy and hence have taken various measures like advanced technology and strict security guidelines to ensure that patient data is always protected.
What do we do with the collected information:
We use the information to provide the services you have requested. Healthcare providers use MedEngage® ( product of Heatho5 Solutions) to monitor, record, and engage patient into self care.
The identifiable patient data is always safe and is never shared with any one without the consent of the patient. It shall be known that Healtho5 Solutions might use un-identifiable patient data (such as symptoms, patient reported health outcome etc) for
The phone call will be made at patients registered number of the patient or caregivers with a sole purpose of collecting patient reported outcome, monitor patients health and provide disease specific counselling and support. However, Healtho5 Solutions shall not contact you or your telephone number if the phone number is registered with DO NOT DISTURB Registry (DND Registry) but will send you SMS suggesting to give us a missed call at specified number to avail the services provided by Healtho5 Solutions. We shall provide you the services only after receiving the missed call and specific, clear and unambiguous oral consent from the patient during phone call.
All the patient reported outcome are stored in the MedEngage® platform for further evaluation and reporting to the designated healthcare provider as per the legal contract. It is to be noted that all the information stored in the system is and shall be reported by the patients during the phone call. The patients are responsible for accuracy of the information provided during the phone call. If patient’s contact and personal information changes or they find any inaccuracies in the data they shall contact Healtho5 Solutions through website, phone number or writing an email at email@example.com. We will make full efforts to make the requested changes in our then active databases as soon as reasonably practicable.
If a patient’s wishes to cancel their account or services, either temporary or permanently with Healtho5 Solutions private limited, with whom they are being engaged directly or indirectly shall contact us through firstname.lastname@example.org. We will retain your previous patient engagement history for the duration defined under the contract with your healthcare provider. We shall not retain such information for longer than is required by lawful contract of agreement with your healthcare provider or any other law applicable at that point of time.
After a point of time your data shall be anonymized and aggregated. It is to be noted that your anonymised data shall be used only for analytics with an aim to improve healthcare delivery.

Patients availing the services both directly and indirectly via Healtho5 Solutions are assured that we are not and will not get involved in any spam activities, ie, Healtho5 Solutions solutions does not intend to sell, rent or otherwise give your contact information (such as, email, phone number etc.) to a third party without your consent.
We at Healtho5 Solutions take your privacy very seriously and hence have implemented best international security policies, protocols, rules and technologies along with operational, technical and physical security measures to safeguard your personal data from unauthorised access, improper disclosure or use, or unlawful destruction or accidental loss. However, if any data is lost due to unforeseeable circumstances or due to unauthorised access from user’s electronic device. Healtho5 Solutions will keep records of telephonic conversation between the healthcare nurses and patients for the purpose to improve customer support and better service delivery to the patients.
All Healtho5 Solutions employees, data scientist, nurses or other employees if any who have access to the patient data or are associated with processing of sensitive personal data are obliged to maintain the confidentiality of patients personal and sensitive information. Healtho5 has put in place procedures and technologies as per industry standards and in accordance with the applicable laws, to maintain security of all personal data from the point of collection to the point of storage and processing.
For the user of Medenegage™: Our system uses two-way authorization ( password and OTP) to ensure that your patients data is always secure. No employee of Healtho5 Solutions will have knowledge of your password. It is important that you should prevent unauthorised access to your password and our device on which MedEngage® platform is used. Make sure you log-out of the platform when you are not using it. Never leave your system open and unattended. Healtho5 Solutions will not be liable for loss of any patient data due to negligence of hospital users using MedEngage® . Healtho5 Solutions uses “masking features” to prevent disclosure of information to unauthorised personnel and share patient information strictly on need-to know basis.
